HOW DO I ACCESS THESE?

First of all, contact the Family Information Zone – FIZ - based next to Newport Bus Station.

Make sure that you pick up an Enrolment form from FIZ to see if your child can access a short break or activity. This will ask you to tick certain boxes and will make it clear if your child is eligible. Once you have decided which short break you want to access, complete the form and hand it in to Lesley Forde, the Information Officer, at FIZ.

Once I’ve filled in the form, then what do I do?

Contact the provider of your choice eg. UKSA or Nettlecombe Farm. They will receive a copy of your enrolment form so that they have your basic information and they may well need additional information from you about your child.

This information will be kept securely on a database and will used to inform the Short Breaks Officer of the need for short breaks and help to plan for services in the future. You can also be kept up-to-date with newsletters and information which may be of interest to you and your child.

Will I have to pay?

£ - denotes that a small charge will be asked for. This will vary according to the activity and the nature of the short break on offer. Often, these breaks have been heavily subsidised and some are real opportunities!

What information do I need to give the provider?

It would be really useful for the provider to know what your child’s key likes and dislikes are. You should mention any health difficulties your child may have and what support they usually require to access school or other activities. If your child needs more time to process questions, this would be helpful to know as well as if your child prefers to sign instead of using language.

It is also important to indicate what support your child needs i.e. whether or not they require help to go to the toilet.
