Description of Service: To carry out adaptations to property to enable the occupier(s) access to the property, around the property and to all essential services through Disabled Facilities Grant following referral by an Occupational Therapist. For a 'self referral form' contact the Occupational Therapy service at St Marys on 01983 534520. For an information booklet send name and address by email to contacts below requesting a booklet, or see the government website- www.communities.gov.uk For minor adaptations less than £1,000 e.g. handrails, bathlift contact Occupational Therapy as above, or your landlord if a Housing Association
