Get Sorted Team June 2008


The Team

YPSM Lead and Team Manager
Funded by the YPSM grant and has a strategic role integrating the work of the Get Sorted Team and linking in with the vulnerable children prevention services.

YPSM Dual Diagnosis Practitioner
A Qualified Therapist funded by the YPSM grant, working with young people with co morbid substance misuse and mental health problems. Strong links with the PCT Early Intervention psychosis Team.

YPSM Tier 3 Worker
Funded by the YPSM grant, with a role in Tier 3 assessment and intervention; specifically linking in with YP and their families on an outreach basis. This role also links in with the A&E department at St Marys Hospital to ensure a fast track response to presentations relating to SM to A&E.

YPSM Tier 3 Worker
Part funded by the YPSM grant and part £10,000 Choosing Health Alcohol funding. They work closely with Looked After Children (LAC), Early Intervention team, YP Hostels and have a specific role promoting safer drinking and working with YP who are harmful/ hazardous drinkers.

YPSM Tier 3 Worker
Part funded by the YPSM grant and part Choosing Health prevention funding to work with schools and respond to referrals for Tier 3 assessment and intervention.

YPSM Tier 3 Transition Worker
Funded by Cranstoun Drug Agency and the Adult Treatment Budget; employed by Cranstoun but works within Get Sorted. The role works with young people above the age of 18 years and supports effective transition from YP to adult services, offering tier 2/3 assessment and intervention.

YOT Worker
Part funded by the YOT grant and also the DIP funding to support increased joint working with PPO and MAPPA processes, increase links with custodial substance misuse services and extended group work and training for individuals .

All workers will undertake Tier 2 work where it is appropriate and brief intervention only is required to prevent prolonged intervention from other agencies/ teams or other services are involved and require support around the YP’s SM.
