The team at the hospital includes one social care practitioner based in A&E and also covering MAU, two staff who cover intermediate care and one staff member who is based at the Earl Mountbatten Hospice.
All of the adult social care staff based within the hospital are part of a multi-disciplinary team. The staff work to particular wards and maintain daily contacts with their wards.
Referrals to social care to support discharge are made by ward staff. This will include both people who are not known and are already known to our service. The team will facilitate timely and appropriate discharges for all referrals accepted. This includes people who also have an allocated community based practitioner. They will liaise with other professionals involved in supporting the discharge e.g. occupational therapy.
