Within this team, staff will work with people with long term conditions and complex needs. This will include safeguarding investigations, continuing healthcare assessments/applications to Eligibility Panel, Mental Capacity Act assessments as well as complicated family situations, supporting carers, working with GPs and primary care staff.
We recognise the vital role that occupational therapy, housing, employment and financial support plays in supporting individuals with a range of long term conditions (including dementia). This service will work closely with colleagues within the Community Occupational Therapy Service, Mental Health Teams and Memory Service, as well as with colleagues across the Council.
