Adults Long Term Conditions Team inc. Memory Service

Memory Service

Local Authority staff are currently co-located with PCT colleagues in the Memory Service.  The service is no longer jointly managed but there remains a commitment to work with our Health colleagues in this service.

Referrals will be received via First Response (Adults) who will undertake the assessment.  We recognise the benefits to people living with dementia and their carers of working closely with colleagues in the PCT to ensure the best outcomes.  There is much work still to do and in recognition of both current and future demographic pressures we must strengthen the capability of our workforce to work with people with dementia to develop expertise and skills in this very important area.  In Adult Social Care we remain committed to the Joint Dementia Strategy recognising the importance of getting the right help at the right time to support people to live well with dementia and their carers, prevent crises and this must be done in partnership.  Access to specialist dementia support must be available to manage the changing needs of people with dementia as the condition progresses.
