Service Description: This service works to support disabled children and young adults up to the age of 25 years and their families with disability related issues. An eligibility criteria applies for this service.

Question: Once I’ve filled in the Short Breaks form, then what do I do?

Answer: Hand this to the activity provider, they will pass this information on to the Short Breaks Officer who will use this to plan future services and demand for short breaks.
