Service Description: This team will help to facilitate timely and appropriate discharges for all referrals accepted.
Answer: Ward staff may refer you to the Hospital Social Work Team for an assessment.. We will consider how we can support you when you are ready for discharge. This may involve a discussion with your family, GP or District Nurse. You will be fully involved in the planning of your discharge and the decisions that are made. Alternatively you may be given information and advice about support available when you leave hospital.
