Application for Transport to and from School for Secondary School Sixth Form Students
Please Note:-
  1. Parents of Sixth form students are required to make a contribution towards transport costs. The contribution is £150.00 for the year or £50.00 payable termly in advance.
  2. Until the completed form is received it will not be possible to organise travel arrangements.
  3. The completed form should be returned (at least 2 weeks before the start of term) to School Admissions, Children's Services, County Hall, Newport, Isle of Wight, PO30 1UD with a cheque or postal order (payable to Isle of Wight Council) for the number of terms you are applying for.
Please do not send cash.
