Please Note:-
  1. Parents of Sixth form students are required to make a contribution towards transport costs. The contribution is �150.00 for the year or �50.00 payable termly in advance.
  2. Until the completed form is received it will not be possible to organise travel arrangements.
  3. The completed form should be returned (at least 2 weeks before the start of term) to School Admissions, Children's Services, County Hall, Newport, Isle of Wight, PO30 1UD with a cheque or postal order (payable to Isle of Wight Council) for the number of terms you are applying for.

* I am appying for a subsidised travel pass concerning my son/ daughters daily journey to school. I understand that:

a) if I move house I must notify School Admissions at County Hall, Newport, immediately as this may
affect my son/daughters's eligibility to receive home to school transport.

b) if I give false information in order to obtain help with transport, the travel pass will immediately
withdrawn and I will be responsible for repaying any fares already incurred.
