for Educational Visits and Journeys |

This form should be used
for all activities which are hazardous, adventurous, or residential on the
Island, mainland or abroad and must be accompanied by all risk assessments pertaining to this visit. |

This application form, when completed by the leader or
co-ordinator, should provide information to the Headteacher and LEA which is
essential before deciding on approval and support.
Not all sections will be relevant to every proposed visit or journey.
This form must be completed by the leader or co-ordinator in charge of a
proposed visit and submitted with the risk assessment to the Headteacher for
approval and Principal Youth and Community Officer, on behalf of the Director
of Education and Community Development for support, at the earliest possible
time.
When approval is given, one copy should be retained in the school or youth
centre office, one copy by the person in charge and a third copy by the LEA.
Any subsequent changes in planning, organisation, staffing etc. should be
communicated to the Headteacher for approval and the appropriate LEA Officer
for support.
PLEASE NOTE: No financial or other commitments should be made before approval
is obtained.
