I apply for a travel pass concerning my childs daily journey to school. I have read the LEA's home to school transport policy set out in the information booklet 'Educating your child'.

I understand that:

a) if I move house I must notify School Admissions at County Hall, Newport, immediately as this may affect my childs eligibility to receive free home to school transport.

b) if I give false information in order to obtain free transport, the travel pass will immediately be withdrawn and I will be responsible for repaying any fares already incurred.
